Please call the office to schedule an appointment. Please be sure to have your insurance information on hand when scheduling. If you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments canceled with less than 24 hours notice.

Billing Procedures

We will bill your insurance directly; our fully computerized claim submission and electronic medical records make it convenient and confidential. We ask that co-pays be paid at the day of service. Patients without insurance may be seen on a fee-for-service basis. Please note that orthotics often are not covered by insurance; routine foot care such as trimming of nails, corns and calluses may also not be covered. Our office accepts most HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier. 

Per Lighthouse Foot and Ankle Center’s financial policy, deductibles/co-pays/coinsurance are expected prior to your visit.
